In order to distract readers from your serial lying, you conflate things that are significantly different.

Observed behavior or bodily sensation is a symptom, not a diagnosis.

When one observes a behavior or experiences a body sensation it is a fact.

The constellation of history, chief complaint, symptoms plus an exam plus any relevant lab or imaging is then considered.

With the requisite knowledge, training, and experience the observations are then ordered into a "differential diagnosis", a ranked set of possibilities.

Only after the stepped process does a rational person select the most likely explanation  as a "diagnosis."

"Consider" the definition that you provided: "…process…through evaluation of patient history, examination, and review of laboratory data.…derived from such an evaluation"

The individual elements of the process are not the diagnosis.

The symptom is not a diagnosis.

The history is not a diagnosis.

The consideration is not a diagnosis.

The process is not a diagnosis.

It is the full process that leads to a diagnosis.

I mentioned only elements observed to be "considered," a cyclicity, and never claimed to perform the complete process and so I never reached or offered a diagnosis. You are the one "way out of your lane."
