Excellent book. TAN used to publish it but apparently it is out of print and there is no indication that they will reprint it.

It may be out of print, but copies can still be found. I purchased one, “new” (though printed in 1993) it was still plastic wrapped. I do not remember the site I purchased it from but it was $40. Amazon had them for $80-180.
