New H7N9 cases confirmed in Sichuan province

Today China reported two more cases of H7N9 avian flu in Sichuan province, according to FluTrackers, an infectious disease tracking message board. The patients are in critical condition. According to translated news reports, no close family contacts have tested positive for the highly pathogenic strain.

In other H7N9 news, Hong Kong's Centre for Health Protection (CHP) today posted an overview of 57 cases reported from the Chinese mainland from Jan 16 to Jan 29. Nine of the cases proved fatal. Thirty-two of the patients had reported exposure to poultry or live-bird markets, while investigations to determine exposure are still under way for 21 patients.

The clear majority of cases involved men (45), and symptom onset occurred from Jan 4 to Jan 29. Jiangsu province reported the most cases (13), followed by Guangdong (10), Zhejiang (8), Hunan (7), Jiangxi (7), Anhui (6), Fujian (4), and 1 case each in Hubei and Sichuan.

"Since November 2016, the Mainland health authorities have already recorded 298 human H7N9 cases thus far. We would also like to remind the public that human H7N9 cases continue to occur in neighbouring Guangdong, and the positive percentage for H7 virus of environmental samples is substantial. We again urge the public to pay special attention to health risks of the places of visit," a spokesman for the CHP said.

Poland reports first H5N5 outbreak; more countries note H5N8

Poland has become the latest country in Europe to report highly pathogenic H5N5 avian flu for the first time, with the virus implicated in an outbreak in Lower Silesian province in the southwest that killed 20 mute swans, officials said today in a report to the World Organization for Animal Health (OIE).

The event began on Jan 27 and has been declared as resolved. Seven countries in Europe have now reported recent H5N5 outbreaks. In addition to Poland, the others are Germany, Greece, Italy, Montenegro, the Netherlands, and Slovenia.

In other avian flu developments, Macedonia reported a highly pathogenic H5 detection in wild birds, while five other European countries reported more H5N8 outbreaks, according to OIE reports.

Macedonia's H5 outbreak began on Jan 13 and involved two mallards found dead at a nature park in Negotino municipality in the central part of the country. Testing was done as part of active surveillance.

Denmark reported four more H5N8 outbreaks in wild birds that began from Jan 7 to Jan 23, affecting different parts of the country. The virus killed three white-tailed eagles and one Eurasian widgeon. Finland reported one more wild-bird outbreak, involving a white-tailed eagle found dead on Jan 22 on the island city of Vardo in the far southwest of the country.

Elsewhere, Greece reported three more H5N8 outbreaks in wild birds, two in Peloponnese region in the south and one in Western Macedonia region in the northwest. The events involved three birds—a mute swan, a graylag goose, and a magpie—found dead on Jan 26. Ireland reported two more outbreaks involving whooper swans found dead in Roscommon and Leitrim counties on Jan 18 and Jan 19, respectively. And Poland reported one more outbreak in backyard poultry, an event that started on Jan 30 in Podkarpackie province in the southeast, killing 44 of 125 susceptible birds.

    Officials reported six more H7N9 avian influenza infections in five different provinces in China, signaling an ongoing surge of activity what will likely make the current fifth wave of illnesses the largest since the virus was first detected in humans in 2013.

    Three of the cases were noted today in an update from Hong Kong's Centre for Health Protection (CHP). The patients are a 54-year-old man from Henan province and two men, ages 55 and 75, in Sichuan province.

    The three other cases were reported in provincial health department notices or China state media reports translated and posted by FluTrackers, an infectious disease news message board. They involve a 75-year-old woman in Zhejiang province, a 47-year-old woman in Hunan province, and a fatal case involving a 3-year-old girl in Yunnan province who apparently was infected in Jiangxi province. H7N9 infections in children are unusual; most illnesses have been reported in adults, especially those who have underlying health conditions.

    China current wave of H7N9 activity began earlier than usual in November with 6 cases, surging to 106 in December and 183 in January. Combined with cases reported so far in February, the country now has at least 302 cases for the current season, which is approaching the record total of 319 reported for the second wave that occurred in the winter of 2013-14.

    H5N8 outbreaks reported in China, Poland

    In the latest avian flu developments today, based on notifications from the World Organization for Animal Health (OIE), China reported its second detection of highly pathogenic H5N8 influenza in wild birds this year, this time involving 20 mute swans found dead near the city of Yuncheng in Shanxi province in the central part of the country. The event began on Dec 21.

    In January, China had reported an outbreak involving domesticated black swans at a zoo in the Hubei province city of Wuhan. Chinese researchers this week described the isolation and analysis of H5N8 linked to a wild bird die-off in May 2016 in Qinghai Lake, a waterfowl breeding and migratory stopover site on the Central Asian Flyway.

    Elsewhere, Poland's agriculture ministry reported one more H5N8 outbreak at a poultry farm, which began on Feb 6 in Lower Silesian province in the southwestern part of the country. The virus killed 1,236 of 22,011 susceptible birds.

    Coming just a week after the first highly pathogenic H5N6 avian flu detection in Taiwan in a wild bird, officials reported four more outbreaks involving poultry, as a handful of countries reported more H5N8 outbreaks and Nigeria reported one more from H5N1.

    Taiwan reports four more H5N6 outbreaks

    News of Taiwan's latest outbreaks came in a series of government announcements over the weekend, according to translations and posts from Avian Flu Diary (AFD), an infectious disease news blog.

    On Feb 11, officials announced that both H5N6 and highly pathogenic H5N2 had been detected in frozen duck carcasses shipped and processed from an area where H5N6 was first detected in a dead gosling found near some poultry farms.

    A report on the findings yesterday from the World Organization for Animal Health (OIE) said the ducks at the slaughterhouse in Hualien County's Yuli township on the eastern side of the island were from one of the farms placed under surveillance after the infected gosling was found and were shipped before the poultry movement ban went into effect. Tests also found highly pathogenic H5N2, a subtype linked to several outbreaks in Taiwan since 2015.

    Authorities destroyed the carcasses and thoroughly cleaned the slaughterhouse, and sampling is underway at the farm that sourced the ducks.

    Yesterday, Taiwan's animal health bureau reported an H5N6 outbreak at a turkey farm housing about 3,000 birds in Tainan on the southwestern part of the island, according to a translated report posted by AFD. Officials said the turkey farm delayed reporting the outbreak, thinking that the illnesses in birds were related to a problem with feed.

    In yet another report today, Taiwanese officials reported two more H5N6 outbreaks, one at a slaughterhouse in Chiayi County and another at Hualien County, according to an AFD post.

    More H5N8 outbreaks, H5 in Bosnia and Herzegovina

    In the latest H5N8 developments, Iran reported 16 more H5N8 outbreaks, which began between Jan 6 and Jan 30and struck wild birds, village poultry, and poultry farms, the OIE said today. The events were mainly concentrated in the north of the country, spanning four different provinces.

    Farm outbreaks affected turkeys, layers, partridges, and broilers. Backyard bird outbreaks also involved ducks, geese, and ostriches. Of 226,443 susceptible birds, the virus killed 20,298, and the remaining ones were destroyed to curb the spread of the virus.

    Affected wild birds included whooper swans and hooded crows found dead at nature parks.

    Elsewhere, Bosnia and Herzegovina reported its first highly pathogenic H5 outbreak as part of wider recent activity in Europe, including many of its neighbors. In a report today to the OIE, the country's trade ministry said the outbreak began Feb 11 in backyard poultry in Prijedor in the northwestern part of the country.

    The virus killed 99 of 148 birds, and animal health officials said the source of the virus is contact with wild species.

    In other European developments:
    •Sweden reported four more H5N8 detections in wild birds, according to a report yesterday to the OIE. They involve mute swans found sick or dead at various locations in Stockholm between Jan 23 and Jan 31.
    •United Kingdom officials today reported their ninth H5N8 outbreak, this time affecting a poultry farm in Britain's Suffolk district, the Department for Environment, Food, and Rural Affairs (DEFRA) said in a statement today. The farm houses 23,000 birds, a number of which died from the virus. The remaining birds will be culled, and officials have placed surveillance and control zones around the farm.

    H5N1 in Nigeria

    Nigeria's health ministry today reported another H5N1 outbreak, this time at a poultry farm in Plateau state in the central part of the country, officials said in an OIE report today.

    The outbreak started on Feb 9, killing 68 of 3,164 layers and pullets of different ages. The rest of the birds were destroyed as part of the outbreak response.

    Nigeria is among of handful of African countries that have reported a recurrence of H5N1 in poultry since 2015. The country has also reported a H5N8 outbreak over the past few months.
