I have been using an unused saddle pad for some time because I don't have a proper kneeler, I just fold it in half and put it on the floor. It's thick enough and soft, but even then after a while my lower back starts to hurt, even if I have my torso straight. My knees are fine, it's just the middle to lower back. I don't know if this would happen just the same in a kneeler with a good cushion, because I have never been in one for any substantial period of time since I have nowhere to go to Mass here.

Yes it will hurt. It's not just the knees it's the whole balancing act ,made worse if you are holding a heavy prayer book.
With the saddle pad it's not thick enough to keep your feet in a natural position,(toes pointing down) you have to twist them to the side.
With a prie dieu your feet are relaxed and you rest your arms on the upper part. This will relieve the strain on your back. But be careful; if the front part you rest your arms on is too high your shoulders will be hunched and give you a back ache anyway (but a good sacrifice).

Yes, and after First Friday Mass, First Saturday Mass and Sunday Mass, I was laid up for a week. Soon afterward I found out that I have multiple vertebral fractures, so I will be the person who has to sit. Please avoid poking me in the back with your missal.

Miserimini is right about a prie dieu.

For lower back pain, if you can find somewhere nearby, daily lap swimming is remarkable.
