  • Does one need to always be dressed very formally when in Church, even if Mass is not going on?

  • What do you mean by "dressed very formally"? It's a bit vague.

  • You are in the presence of the Blessed Sacrament, and so, you would want to be dressed modestly, and not in sloppy clothing. That's my view.

But let your speech be yea, yea: no, no: and that which is over and above these, is of evil.

  • Yes.  It is not necessary to wear a suit and tie or the female equivalent in formality to every Mass.  Modesty, clean clothing, and respect are what is needed.

  • If you were to travel to a far-off country to be in the presence of a very holy Catholic king, how would you dress?  Add to that, a king who underwent much suffering for the love of his people.  I'm not saying that I always dress in my Sunday best, but maybe we should.
But one thing is necessary. Mary hath chosen the best part, which shall not be taken away from her.

Yes, provided you are dressed modestly, well groomed, and show respect, Our Lord will be more concerned with the disposition of your heart than what what you're wearing.

I did not mention modesty and cleanliness, but I should have.  They are essential.
