Scratch the porters (doorkeepers and bell ringers)  I guess using porters is just a fantasy.
I guess it never was the ideal.

Does the Blessed Sacrament reside there constantly or only during Mass?

In the home chapel, no.
In the organized chapel, yes.
I would say it does make a difference. If Our Lord is Present in the Tabernacle, particular care and protocol must be preserved. Women must cover their heads, there must be no more noise than absolutely necessary, no banter or speaking, genuflections must be done. Reverence must be maintained, not just respect. 
As for the baptismal font, I don’t know. It is located outside the sanctuary in both places.
