I should also add, "Has the person made a confession in the past year?  Has he/she received the Body of Christ in the last year?"

Exactly, the minimum the Church imposes are these 2 things (along others of course)......not get do either is a mortal sin in and of itself....

6)  Does the individual profess submission to the Pope, the Vicar of Christ on Earth, and to those bishops who also profess submission to the Pope?

Knowingly receiving from heretics and schismatics is a mortal sin.  Not receiving sacraments when they are not available is not a mortal sin.
