So when and where did the priest see her for the first time?  at the communion rail?  Not the appropriate time to admonish/instruct/center out anyone on their first time visit.
If you saw her before Mass did you offer her a sweater or ask if she had something to cover with? Or if the young man was a regular, did you take him aside and encourage him to rectify her error?
Not every situation can be resolved to a charitable/satisfactory outcome on sight.
Remember, the bible says to admonish the sinner once, 
then admonish the sinner with another witness,
Then take the problem to the priest.

I’ve seen refusal of entry into the chapel, and one priest had a man at the door on Sundays, whose job was to turn immodestly dressed people away. 

With one priest I knew, if he noticed during Communion or Mass that there were newcomers not dressed properly, he would sacrifice his long prayer time after Mass and go find them as soon as possible, ensuring he can speak with them, catechize them and so on. 

If a priest sees an immodestly dressed woman at Mass and she is there for holy communion, it’s not that hard for him to gently whisper “come and see me after Mass” and simply not give her the sacred Host.

Some of those are great suggestions, but I would add that the problem with immodest dress is twofold: one, the blatant immodesty which is an occasion of sin to others and disrespectful to God; two, the corruption of the heart to presume to enter a Church either without caring, or without inquiring what the dress code is. It seems people inquire about this with which restaurant they want to attend, or what is appropriate for a certain party.

The Holy Sacrifice of the Mass is infinitely more valuable than those things, and this is what people do in preparation? It is a solution for the scandalized men in that chapel for the woman to be given a sweater, coat, etc., but that does not solve the sin in her heart, which would be resolved by a sincere confession. Before receiving the sacraments, such a person would probably require some Catechism as well.

As I said above, it is easy for a priest at the communion rail to simply say “come and see me after Mass, God bless you” and to not give Our Lord away to such a heart. His honour comes before our salvation.
