1-5 suffice for the valid administration of the Sacrament, and thus the laver.
6 is also required for justification from the Sacrament.
If 1-5 occur without 6, once 6 is added, the Sacrament is not repeated and the soul then enters a state of justification (and of course Confession would be required for the sin of having received the Sacrament of Baptism without the proper dispositions).
I hold that the laver refers to the valid administration of the Sacrament, and that's why Trent cites Our Lord's teaching (echoed in Titus 3:5) that the laver AND the Holy Ghost are required for justification by the Sacrament.
