I have signed a docuмent very similar to this one before marriage. It is merely to see that you are in good faith, because they know that, whatever they decide, you can easily get an annulment at the Novus Ordo courts.
I see no problem with the form.
The problem, as usual, is that everything is a secret in the SSPX, and you are the bad person if you start asking questions, however legitimate they are. They just expect you to wait forever until they feel like giving you a response. You could try to contact you district superior, but don't expect much.
If they have a single group of people to work on this kind of request for all the SSPX faithful, it is really going to take some time, since they probably get a considerable amount work from all over the world.
