I know that medicine doesn't break the fast, but I still have a concern.
I recently got a tongue tie release and as part of my stretches I need to do I need to use coconut oil (more like a butter than a liquid) on the lesion. It's only a small amount but would this constitute breaking the fast?

No. It’s for medicinal purposes, not because you’re eating or drinking coconut oil for nourishment. If you overdo it, you’re in for an unpleasant surprise.
